Why Your Squat Technique Breaks Down the Heavier It Gets
Depth, bar path, and knee position aren't three separate problems. Most of the time they're the same one, showing up in three places.

Why squat technique breaks down under load
Squat technique usually breaks down for two reasons: the bar drifts off balance over the middle of the foot, or bracing loosens enough that the torso can't stay rigid. Depth, knee position, and bar path are downstream symptoms of those two failures, not separate problems.
Most lifters can hold a decent-looking squat at a weight that doesn't ask much of them. Add load, and the same lift starts falling apart in the same few places: the knees drift inward, depth quietly gets shorter, the torso pitches forward earlier in the descent than it used to. It rarely feels like one clean break. It feels like the whole lift getting harder to control at once, and most lifters describe it as a mistake in their setup rather than the balance-and-bracing problem it usually is.
The real cause of common squat faults
Mark Rippetoe's Starting Strength frames the squat as a whole-body movement organized around keeping the bar over the middle of the foot, from the first inch of the descent to lockout. Dan Austin and Bryan Mann's Powerlifting goes further into how that balance point interacts with hip and ankle mobility, and how those joint relationships change as the bar gets heavier. Sullivan and Baker's The Barbell Prescription makes a related point from a different angle: teaching a movement pattern well, at any training age, is about giving the body a repeatable, stable path to find, not about telling it to try harder.
That lines up with what the American College of Sports Medicine's resistance-training guidelines describe as maintaining joint alignment under increasing load, not just moving more weight from one session to the next.
A squat with balance and bracing solved is a squat where the working muscles don't have to compensate for anything else. When bracing is inconsistent, or the bar drifts even slightly off the mid-foot line, the body finds the nearest stable position instead, which is almost always a shorter, less controlled version of the lift. Under fatigue, that shortcut gets picked more often, because it's the path of least effort to stay balanced.
Common squat technique faults to watch for
A few patterns account for most of what a coach sees on video review. The knees track fine near the top of the descent, then drift inward as depth increases, usually because the hips are running out of the range they've built to control that position. The bar creeps forward off mid-foot when a lifter starts thinking about standing up before the descent is finished, shifting weight onto the toes a beat too early. Depth quietly shrinks as a set goes on, because bracing fatigues before the legs do, and a shallower rep is an easy way to stay upright.
A related fault shows up specifically on the way back up: the hips and back rise before the shoulders do, so the bar path breaks and the lift turns into something closer to a good morning than a squat for that last portion of the rep. This is one of the faults Starting Strength spends real time on, because it usually means the lifter's hip extensors are firing before the whole system is braced to use them, not that the hips or back are somehow too strong for the rest of the lift.
None of these show up because someone doesn't know what a good squat looks like. Knowing the shape and holding it under a heavy, fatigued bar are different skills, and closing that gap is what technique coaching is for. The same fatigue-driven drift shows up in the bench press and deadlift, just at different joints.
Why squat faults often appear during a program transition
Rippetoe's novice linear progression, the model most beginner programs still borrow from, calls for adding weight to the squat every single session. That means squat technique gets tested against a heavier load than the lifter has ever handled, over and over, with almost no repeated practice at any one weight. Faults tend to show up right at the edge of what a lifter can currently hold together, because that edge keeps moving forward every workout.
The pattern changes again once a lifter moves into an intermediate model like the Texas Method, which typically drops the heavy squat down to once a week instead of nearly every session. Now the question isn't just whether technique holds at a new heaviest weight, it's whether it holds after a week away from a truly heavy squat. Coaches watch for both patterns, because a fault that had disappeared under frequent practice can quietly come back the moment a program changes how often the lift gets trained heavy.
Technique work never really stops
A cue that solves knee cave on Tuesday can stop working on Friday, once the load or the fatigue changes the problem underneath it. Coaches keep checking squat technique at every level, from a first session to a state-record attempt, because the lift keeps testing whatever changed since the last time someone looked at it. Technique doesn't stay fixed once you learn it. It degrades under fatigue, under a new personal best, under a heavier training block than you're used to. A compensation that's invisible at 60% of your max shows up the moment the bar gets heavy enough to remove your margin for error. That's not a beginner problem. It's why even strong, experienced lifters still get their squat reviewed.
A video can tell you what a fault looks like in general. It can't tell you whether you have that fault, on your body, on that specific rep. Most lifters who try to self-correct from cues alone end up trading one compensation for another, because they're guessing at what's happening on their own lift without anyone actually watching it.
